Community Connections Program

In Community Connections we walk alongside people to explore their strengths and create goals that widen and increase their connection with community.

People receive one-to-one and group support over 12 weeks, to create meaningful new social connections and explore any barriers making connection difficult. The program is flexible and tailored to each individual’s need, and may include:

  • helping people to get involved in local community activities

  • finding new support groups

  • meeting their neighbours

  • volunteering

  • learning new skills

  • finding opportunity to share existing skills

If you are aged between 18 and 64 and not receiving NDIS, CCP is available at no cost to you, funded by the Department of Human Services.
